Leopard Snake Skin Death

I have a 2 year old leopard snake skin Discus("Male"). Recently he has been getting skinnier and skinnier. I assumed that I haven't been feeding the discus much. So I upped my feeding assuming he will be back to normal. Two days ago, he was outside of the discus group at the top of the tank alone.

Yesterday I saw him at the far right of the tank by himself breathing really slowly. Today he was dead.... I don't know what has caused this. I have asked a friend regarding it, and they said it could be caused by worming.

If it is worms, do I treat the tank or do I treat the discus individual. What would I need to get?And how would I know? Please help, I do not want to loss my babies.
