I can recall one incident in a large company a few years back where a 9" grinder wheel (thin cut off type) shatter and a piece hit the operator in the heart killing him, also had a mate who's a sheet metal engineer had a 5" wheel do the same and embedded it's self in his forearm doing some major damage to his tendons. I've had a few wheels let go but luckily have never been hit by any, Most common cause of grinder wheel accidents is people using cut off wheels and loading them up.
